Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on September 7, 2023 from approximately 11:25 am to 2:30 pm. There were a total of 16 children present, ranging in ages from 2 to 4 years, with 5 staff supervising.

A total of 5 children records and 4 staff records were reviewed. During the inspection, children were observed during rest time and playing with toys. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children.

A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records and interview, the vendor did not ensure all staff who work directly with children have within 90 days of the date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Based on staff records review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department.

Evidence:
1. The record for Staff #2 (date of employment: 5/17/2021) contained documentation of the Virginia Annual Health and Safety Update Course dated 6/15/2021. There was no documentation that the course has been completed since 6/15/2021.
2. The record for Staff #3 (date of employment: 3/23/2015) contained documentation of the Virginia Annual Health and Safety Update Course dated 7/15/2015. There was no documentation that the course has been completed since 7/15/2021.
