During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 10/10/2024 and 10/17/2024 and completed on 10/17/2024 in response to a complaint received by the licensing office on 10/8/2024 relating to allegations of supervision. The inspector reviewed one children?s record and two staff records on-site on 10/10/2024. This inspection included document review, tour of the facility and interviews.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/30/2024. A POC submitted after this date will not appear on the public website.
Violations
3Staff #1 TB was dated two months prior to date of employment and has been employed for three months.
A classroom consisting of 10 school-aged children were under the supervision of Staff #1, a teacher?s aide, when a physical altercation occurred between two children. A program leader was not present in the classroom during the time of the physical altercation.
Staff #1 completed the injury report and was listed as the only present staff during the injury. A program leader was not identified as providing supervision for the children on the date of the occurrence.
Staff #1 left the school-age classroom and went across the hall to the kitchen to gather snack for the children. While Staff #1 was out of the classroom, a physical altercation occurred between two children, ages 7 and ages 8.
Staff #2, passing the classroom observed the altercation and separated the children and informed Staff #1 of the incident. The physical altercation resulted in bruising to Child #1?s face, specifically around his eyes.
