Inspector Notes
An unannounced monitoring inspection was conducted today April 17, 2023, to determine the centers compliance with licensing standards. The licensing inspector was on site from approximately 1:50 pm until 4:15 pm. The Director assisted with the inspection. There were 33 children present, ranging in ages from 16 months to five years, with five staff supervising. Children were napping upon the inspector?s arrival. Later children had snack and played outside in the playhouses, on trikes and with other outdoor toys. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Staff interacted with
children in a positive manner. A total of five child records and seven staff records were reviewed. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

7Evidence:
1. The record for staff #6 contained the results of a fingerprint national criminal record check dated 04/11/2018, exceeding the requirement to renew every five years.
2. Staff #2 was observed working during the inspection.
3. The director confirmed a request to update the fingerprint national criminal record check
had not yet been requested.
Evidence:
1. The record of staff #5 contained documentation of a tuberculosis screening dated 01/12/2021, exceeding two years.
2. The record of staff #6 contained documentation of a tuberculosis screening dated 02/15/2021, exceeding two years.
Evidence:
The record of staff #1, employed 11/18/2022, did not contain documentation of completion of the Virginia Department of Education-sponsored orientation course, exceeding 90 calendar days from employment. The director stated staff #1 had nor yet completed the course.
Evidence:
At approximately 4:10 pm, the licensing inspector observed 23 children (ages two years to five years) on the playground with staff #1 and staff #6 supervising. The required staff-to-children ratio is applied to the youngest child in the group and is applicable to the entire group. The required staff-to-children ratio is one staff member for every eight children present.

At approximately 2:00 pm, the licensing inspector observed 15 children (ages three years to five years) with staff #6 supervising. The required staff to children ratio is one staff member for every ten children.
Evidence:
Staff #6 was observed working alone in a classroom with children. The record of staff #6 contained documentation of CPR certification that expired on 01/22/2023. The director confirmed the CPR certification for staff #6 has expired and a renewal certification class had not been completed.
