Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on April 17, 2023. There were 33 children present with five staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The record of staff #2, employed 07/19/2021, did not contain documentation of completing
the department?s annual health and safety update course for the 2022 year.
2. The record of staff #5, employed 01/22/2021, did not contain documentation of completing
the department?s annual health and safety update course for the 2022 year.
obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment.
Evidence:
1. The record of staff #1, employed 11/18/2022, did not contain documentation of current CPR
certification, exceeding 90 days from employment. The director stated staff #3 had not obtained certification in CPR.
2. The record of staff #5, employed 01/22/2021, contained documentation of CPR that had expired on 01/22/2023. The director stated staff #5 had not renewed her CPR certification yet.
3. The record of staff #6 contained documentation of CPR that had expired on 01/22/2023. The director stated staff #5 had not renewed her CPR certification yet.
