During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on March 27, 2025, and completed on March 27, 2025, as a part of the licensure period. The on-site inspection began at 1:50 pm and ended at 3:45 pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 5 staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 8, 2025. A POC submitted after this date will not appear on the public website.
Violations
5Staff #5 was observed sitting in the hallway between classroom A and classroom B during rest time to supervise both rooms. Due to the placement of staff #5 in the hallway, they were unable to visually supervise all the children in classroom B, that had nine children ages 3-5 years. Staff #5 had limited visibility into classroom A, which had two children, 2 years and 20 months in age, due to lack of lighting in the room. There was no additional staff in classroom A or classroom B.
Staff #4 was observed changing a diaper of a child, 2 years in age, on a mat covered with an absorbent cover. Diapers were being changed on the mat that was on the floor. There was no changing table or countertop for changing. Staff #4 was then observed placing used diapers into a trash bag that was sitting on the floor.
