During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on September 4, 2025, and completed on September 4, 2025, as a part of the licensure period. The on-site inspection began at 1:00 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and six staff. The inspector reviewed five children?s records and six staff records on site. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for three weeks, did not have documentation the central registry was requested prior to employment. The results of the central registry were received 08/21/2025.
At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening. An updated tuberculosis screening for staff #2 was due four months ago. The updated tuberculosis screening has not yet been completed.
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #2 obtained 10 of the required 16 hours of training between the period of September 1, 2024, through August 31, 2025.
