The center is required to have employees undergo a background check every five years. 1. The most recent national criminal fingerprint background check for staff #3 was overdue by one month. 2. The most recent central registry check for staff #3 was overdue by 14 days. There was no documentation that the national criminal fingerprint background check and central registry check had been requested.

The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of the implementation of the required testing plan.

The center shall ensure disposable diapers are disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff in the two-year-old classroom were observed placing soiled diapers in into an open trash pail. The director stated there was no hands-free diaper pail on site.
