Inspector Notes

An unannounced inspection was initiated on June 20, 2024 and completed on June 20, 2024 in response to a complaint received by the licensing office on May 8, 2024 relating to allegations of supervision, staffing and physical plant. The inspector reviewed one child?s records and four staff records on site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports part of the allegation regarding staffing; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Based on observation and interview, the center did not ensure that when children are in ongoing mixed age groups, the group size applicable to the youngest child in the group shall apply to the entire group.
Evidence:
1. During the inspection there were 13 children ages 16 months to 12 years old in the same group with two staff. The maximum group size should have been 12 children.
2. Staff #1 confirmed that the staff did not follow the correct group size requirements.
