An unannounced inspection was initiated on October 8, 2024 and completed on October 18, 2024 in response to a complaint received by the licensing office on September 9, 2024 relating to allegations of Staff Qualification and Physical Plant. The inspector reviewed 1 staff record on-site on October 18, 2024. This inspection included document review, tour of the facility and interviews. The preponderance of evidence gathered during the investigation does not support the allegation therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
