Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 09/24/2025, as a part of the licensure period. The on-site inspection began at 11:04 AM and ended at 12:28 PM. The inspector reviewed compliance in the areas listed above. There were 23 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the names, addresses, telephone numbers and employment information for each parent or guardian having custody of the child.
The record provided for child 3 had a father indicated on the birth certificate but the record contained no additional information regarding the father.
Plan of Correction: Provider response:
All children's records will be reviewed to ensure that required information regarding each parent/guardian is provided in the record.
