Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on July 21, 2021, and concluded on July 22, 2021 approximately 9:09 am to 10:00 am. The director was contacted by telephone and a virtual inspection was conducted. There were 32 children present, ranging in ages from two to four, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-210-A
Based on staff records review, 2 out 2 records, the center did not obtain documentation program leaders 18 years of age have fulfilled a high school program completion or the equivalent.
Evidence: Staff #1 (date of employment 2/27/2017) did not have documentation in record for review during the inspection as a program leader fulfilled a high school program completion or the equivalent.
Plan of Correction: I made a copy of staff diploma and add to her file.
Standard 22VAC40-185-70-A
Based on staff records review, 2 out of records, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
Evidence: Staff #2 (date of employment 5/25/2021) did not have two or more references in record for review during the inspection.
Plan of Correction: I called her 2 reference on 7-27-21 and add to the file.
