Inspector Notes

A monitoring inspection was initiated on 8/9/2022. There were a total of 30 children with 8 staff members present. Children were observed during nap time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, the center did not obtain documentation of additional immunizations once every six months for children under the age of 2 years.

Evidence: Record for Child #1 did not contain documentation of additional immunizations once every six months. Child is 2.5 years old, and there is only record of their 6 month immunizations.
Plan of Correction: Center will request updated immunization records from parent.
