A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 was due for an updated screening one month ago, Staff #3 was due for an updated screening over two months ago, and Staff #5 was due for an updated screening over five months ago. The updated screenings have not yet been completed.
Plan of Correction: The center will obtain TB screenings from the staff members.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, #2, #3, #4, and #5, did not contain the work phone numbers for both parents.
