Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on July 21, 2021, and concluded on July 22, 2021 approximately 8:22 am to 9:15 am. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from one to three, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-100-A
Based on children's records, 2 out of 2 records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment 6/08/2021) did not have a physical examination in file to review during the inspection.
Plan of Correction: Mom sent the physical but it was missing the doctors signature, so mom resent a new form with the docotrs signature and was placed in her file.
