Inspector Notes

A renewal inspection was initiated on 12/16/2022 and concluded on 12/16/2022. There were a total of 7 children (including provider's 3 children) with 1 staff member present.

Children were observed individual play and screen time for musical movement. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
Based on staff records review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first
screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Staff #1 tuberculosis screening expired on 10/4/2022.
Plan of Correction: An appointment has been scheduled for next week.
Standard 8VAC20-800-700-C
Based on medication records review, when an authorization for medication expires, the provider did not notify that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days shall
be taken to a pharmacy for proper disposal.
Evidence:
1. In a locked box, Child #1 authorized medication expired on 9/2022.
Plan of Correction: I will get an updated Epi-pen from the parent.
