Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 5/5/2025 and completed on 5/5/2025. The on-site inspection began at 10:55AM and ended at 12:25PM. The inspector reviewed compliance in the areas listed above. There were 12 children present, in addition to the Provider's child and 3 caregivers; the point total was 31. The inspector reviewed 4 children?s records and 3 caregiver/household member records. This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. For any questions regarding this inspection, please contact stephanie.salvatori@doe.virginia.gov.
Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service.
The Provider could not find the search results for Caregiver A who has been employed for over 1 year.
Plan of Correction: I will have Caregiver A redo the central registry tomorrow, as my old email crashed and I don?t have the emailed copy anymore. I will try to call the CRS office to see if they might be able to look up the records and email a new copy.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
The Provider did not have documentation of a physical examination for Child A. Child A has been enrolled for approximately 4 months.

The provider shall obtain the parent's written acknowledgement of the receipt of the information in this section.
The provider did not obtain the parent's written acknowledgement of the receipt of required information for Child A's Liability Insurance Declaration and Child B's Information for Parents document.

Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the identity and age of the child.
The provider did not obtain the required verification of the identity and age of the child within seven business days of the child's first day of attendance. Child A has been enrolled for approximately 4 months.

The provider/person viewing the proof of identity, for Child B, did not sign or date when the proof of identity was seen.

Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
The Provider did not have immunizations for Child A. Child A has been enrolled for approximately 4 months.
