Plan of Correction: Provider did have two references for Staff #2 but the forms were accidentally miss placed in another employees file. Provider corrected her mistake and placed the missing files with Staff #2 files.

The record for Child #4 was missing the address and telephone number for one designated person to contact. The child was present during the inspection and had been in care for 10 months.
