Inspector Notes
An unannounced Monitoring Inspection was conducted on 4/12/22 from 8:05am to 10:20am. There were 25 (Infants to School-Age) directly supervised by 7 staff. The physical plant, 3 staff records, 5 children?s records, 9 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, listening to stories, playing outside, working on the computer, and interacting with staff. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations
4Evidence: observed that there was a broken cabinet door in the Preschool One room.
Evidence: observed in the Explorers Room (school-age) an unlocked cabinet with a sanitizing solution and disinfectant spray.
Evidence: the facility did not have a complete written policy containing the topics listed above.
Evidence: reviewed 11 injury reports and found that 2 did not have documentation of time parents were notified.
