Inspector Notes

An unannounced Monitoring Inspection was conducted on 4/12/22 from 8:05am to 10:20am. There were 25 (Infants to School-Age) directly supervised by 7 staff. The physical plant, 3 staff records, 5 children?s records, 9 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, listening to stories, playing outside, working on the computer, and interacting with staff. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on observation, the facility did not ensure that areas and equipment of the center are maintained in a clean, safe and operable condition.
Evidence: observed that there was a broken cabinet door in the Preschool One room.

Based on observation, the facility did not ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed in the Explorers Room (school-age) an unlocked cabinet with a sanitizing solution and disinfectant spray.

Based on review, the facility shall develop written procedures for prevention of shaken baby syndrome, abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
