Inspector Notes
An unannounced monitoring inspection was conducted on 4/10/23 at 10am. There were 76 (infants to school-age) directly supervised by 13 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
3Evidence: reviewed 4 staff files and found that Staff 1 did not have documentation of orientation training.
Evidence: observed that there was peeling paint in the lower area of the fence on the Thumbs Up playground.
Evidence: reviewed 8 injury reports and found that 2 did not have documentation of the time parents were notified of the injuries.
