During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/3/24. The on-site inspection began at 12:40 pm and ended at 2:55 pm. The inspector reviewed compliance in the areas listed above. There were 63 children present and 10 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis test/screening . The documentation shall have been complete within the last 30 calendar days of the date of employment.
Documentation of the tuberculosis test for staff A was dated one year prior to employment.
Plan of Correction: We have located a tuberculosis test/screening in October 2024 for this staff.
Standard 8VAC20-780-550-E
Shelter in place procedures shall be practiced a minimum of twice per year.
The last shelter in place procedure/drill was practiced on 9/29/23.
Plan of Correction: We will do a shelter in place drill this month and make sure we are conducting at least two drills annually.
