Each staff member shall submit documentation of a negative tuberculosis screening which shall be signed by a physician, their designee or an official with the health department and the documentation shall have been completed within the last 30 calendar days of the date of employment.
The tuberculosis test for staff A, B, and C were not signed by a physician/designee. This was overdue by 5 weeks for staff A, by 11 months for staff B and by 5 months for staff C.
The tuberculosis test for staff D was obtained 2.5 months prior to employment.
