During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 08/25/2025, as a part of the licensure period. The on-site inspection began at 8:57 AM and ended at 10:40 AM. The inspector reviewed compliance in the areas listed above. There were 17 children present and 4 staff. The inspector reviewed 4 children?s records and 3 staff records on-site.
This inspection included:

? observations.
Violations
5There were pokeberry plants with berries, toxic to humans, growing through the fencing on the large outdoor playground.
Maintenance was immediately notified to remove the pokeberry plants. All staff will be reminded to be vigilant when inspecting the playgrounds for potential hazards.
There was a container of lotion and a container of hand sanitizer, both with warning labels, on a desk at the end of the central hallway.
Lotion and sanitizer were re-located to a locked location during the inspection. All staff will be reminded of those substances that are potentially hazardous and that they must be kept in locked areas at all times.
Children, in a mixed age group of infants to three years, were observed eating food off the floor and reaching into and eating each other's food without staff intervention or assistance.
All staff will receive training regarding classroom food service to include supervision, safety, procedures and sanitation.
There were ten children ages infant to three years in the infant classroom with only two staff until the inspector notified the coordinator that a third staff person was needed.
The morning coordinator will be re-trained regarding ratio compliance and classroom coverage in the absence of staff. All staff will received training regarding required ratios and procedures to notify administrators should there be
non-compliance.
Each center shall maintain and keep at the center a separate record for each child enrolled which contains the names, addresses, telephone numbers and employment information for each parent.
The record provided for child 3 indicated N/A for the father with no additional information, however a father was documented on the birth certificate.
All children's records will be reviewed to ensure that all information is complete to include required information regarding parents/guardians.
