During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 9/24/2025, as a part of the licensure period. The on-site inspection began at 2:20 PM and ended at 3:35 PM. The inspector reviewed compliance in the areas listed above. There were 10 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

The child day center obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
According to the sworn disclosure statement signed by staff 3, staff 3 has lived in two states other than Virginia in the past five years. No background checks were obtained from those states.
Plan of Correction: Provider response:
Background checks (criminal record checks, child abuse registry searches and sex offender searches) will be obtained from those states in which the staff person has lived within the past five years. In the interim, the staff member will not be in the facility or caring for children. In the future, sworn disclosure statement will be carefully reviewed with staff to enjoy that the staff member has not lived in any other state within the past give years. If so, background checks will be obtained prior to employ.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a leak-proof storage system that is either foot-operated or used in such a way that neither the staff member's hands nor the soiled diaper touches an exterior surface of the storage system during disposal.
A staff member was observed disposing of a child's soiled diaper by using her hands to open the lid of the diaper disposal system.

Appropriate diaper changing procedures will be reviewed with all staff.
