Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews) and a virtual tour of the program. A monitoring inspection was initiated on 8/12/2021 and concluded on 8/12/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 4 years to 7 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of caregiver and household member records, the provider has not obtained a current Report of Tuberculosis Screening form, every two years from the date of the first screening .
Evidence: Caregiver #1 and Household member #2 TB screening is dated 7/13/2019
Household #3 TB screening is dated 6/1/2018.
Plan of Correction: We will get our TB screening updated.
