During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 01/14/2026 and completed on 01/14/2026. The on-site inspection began at 8:55 am and ended at 10:00 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregivers; the point total was 8. The inspector reviewed 3 children?s records and 2 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 01/27/2026. A POC submitted after this date will not appear on the public website.

The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended.

Caregiver #1 and Household Member #1 had TB tests that were expired by 3 months.
