Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 12/20/2024. The on-site inspection began at 9:30am and ended at 11:00am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 4 children present and 3 caregivers; the point total was 11. The inspector reviewed 4 children?s records and 5 caregiver and household member records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Annually, the provider shall review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. The last review of the emergency contact information in the record for Child #2 was 15 months ago. The annual review of this information was 3 months overdue.
Plan of Correction: The provider has reviewed the emergency contact information with the parent and the record has been updated to include the parent signature.
