During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/30/25. The on-site inspection began at 10:00am and ended at 11:30am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 4 children present and 3 caregivers; the point total was 12. The inspector reviewed 5 children?s records and 4 staff and household member records on-site on 6/30/25. Additional records were viewed electronically on 7/1/25. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-350-E
Hot water at taps available to children shall be maintained within a range of 105?F to 120?F. The hot water at the bathroom sink measured 126 ?F.
Plan of Correction: The provider responded: The temperature of the water heater was turned down following the inspection on 6/30/25.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. Child #1, 23 months old, last documented immunization was over 10 months ago.
Plan of Correction: The provider responded: The updated immunization record was requested and received following the inspection on 6/30/25.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated. The posted menu was not dated for the current week. The menu was dated 3/3/25-3/7/25.
