During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 11/12/24 with a local agency in response to a complaint received by the licensing office on 11/8/24 relating to allegations of inadequate supervision. The inspector reviewed 1 child records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to supervision and injury reporting and are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
(Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Evidence:
On the day of inspection, at approximately 5:30pm, Child #2 (age 8) was found alone in the cafeteria behind a closed door. Staff stated they were not aware the child had left the gym.
Plan of Correction: Procedures are in place for alerting other staff members for assistance when priorities shift/situations arise.
Standard 8VAC20-780-550-P
The Center is required to ensure that the written record of children?s injuries contained the date when parents were notified; any future action to prevent recurrence of the injury; and Staff and parent signatures or two staff signatures.

Evidence: The injury report written for Child #1 was missing the date the parents were notified, future action to prevent recurrence of the injury, and two signatures.
Plan of Correction: Current accident report is dated, will include date in parent notification box moving forward.
Standard 8VAC20-780-80-B-2
The center is required to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: The center did not inform the licensing inspector within 2 business days after an incident requiring medical treatment.
Plan of Correction: Proper notification and documentation will be submitted within the timeframe when needed.
