Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 5/19/25. The Department issued a Notice of Intent to place licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 12/23/24 as a result of high-risk violations. The on-site inspection began at 6:55am and ended at 7:25am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 4 staff. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
