Inspector Notes

A monitoring inspection was conducted today. There were 8 (22 points) children present with 3 caregivers present. The physical plant, children's records, caregiver records, household member records, the outside play area, cribs, bottles, first aid kit, menu's and the emergency preparedness plans were reviewed. Standard 8VAC20-800-60-A
Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #1's record did not contain documentation of liability insurance form, medication policy or emergency plan.
Child #3's record did not contain documentation of a second emergency contact, liability insurance form or the emergency plan.
Child #5's record did not contain the phone number for the physician.
Plan of Correction: I will get the information from the parents and place in children's files.
Standard 8VAC20-800-690-D
Based on observation the provider did not ensure that the diapering surface will be nonabsorbent and washable.

Evidence:
The changing pad had holes in it and therefore was not nonabsorbent.
Plan of Correction: I will purchase a new changing pad for the changing table.

Based on observation, the provider failed to ensure that all required supplies were contained within the first aid kit.

Evidence:
The first aid kit did not contain an instructional manual.
