Inspector Notes
An unannounced renewal inspection was conducted on January 18, 2023. There were 9 children present with 3 caregivers supervising. The home, children's records, caregiver records, household member records, attendance records, training records, emergency evacuation records, menu's, cribs and the first aid kit was reviewed. There were no medications on site. The children were observed napping, singing songs and preparing to go outside. Areas of non-compliance were cited on the Violation Notice. Standard 8VAC20-800-100-A
Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #5's first day of attendance was 9/7/21, physical examination documentation is dated 5/9/22.
Plan of Correction: I will ask parents for the document. All new children enrolled will have a physical records by the 30th day of attendance.
Standard 8VAC20-800-210-A
Based on a review of training documents, the provider failed to ensure that all caregivers shall obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
Caregiver #1 and #2 did not obtain 16 hours of training for 2021-2022.
Plan of Correction: We will all have required training hours for 2022-2023.
Standard 8VAC20-800-280
Based on observation, the provider failed to ensure that potentially poisonous substances such as but not limited to cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, petroleum distillates shall be stored in areas inaccessible to children.
Evidence
On the bathroom counter, deodorizer was in an area accessible to children.
Plan of Correction: Hazardous substance was removed during inspection.
Standard 8VAC20-800-590-A
Based on observation, the provider failed to ensure that infants shall be placed on their backs when sleeping or napping unless otherwise ordered by a written statement signed by the child's physician.
Evidence:
Child #1 was asleep on his stomach. I asked the provider if he was able to turn over on his own, her response was no.
Plan of Correction: When infants are put down for nap, they will be placed on back.
Standard 8VAC20-800-60-B
Based on a review of children's records, the provider failed to obtain all required documents for each child enrolled.
Evidence:
Child #4's record does not contain documentation of the name, address and phone number for one designated person to contact in case of an emergency if the parent cannot be reached.
Child #2, #4 and #5's records do not contain documentation of the review of the child's emergency contact information as required by 8VAC20-800-960-A.
Plan of Correction: I will get the information from the parents and place in child's file.
Standard 8VAC20-800-680-A
Based on observation, a caregiver failed to wash their hands with liquid soap and warm running water after diapering a child.
Evidence:
Caregiver #2 did not wash hands after diapering a child.
Plan of Correction: I will remind caregiver's that they must wash hands after changing diapers.
Standard 8VAC20-800-890
Based on observation, the provider failed to ensure that when food is brought from home the food container shall be clearly labeled in a way that identifies the owner.
Evidence:
On the counter were two containers, one with dry cereal and one with fruit that were not labeled in a way that identified the owner.
Plan of Correction: I will remind parents to label all food brought from home and I remind caregiver's to label food from home if parents forget.
Standard 8VAC20-800-90-A
Based on a review of children's records, the provider failed to obtain documentation that the child has been adequately immunized according to the Code of Virginia and applicable State Board of Health regulations, prior to or by the first day of attendance.
Evidence:
Child #4's first day of attendance was 9/27/21 and immunization records are dated 9/29/2021.
Child #5's first day of attendance was 9/7/21 and immunization records are dated 05/09/22
Plan of Correction: I will get immunizations on the first day for all new children enrolled.
