Inspector Notes
A monitoring inspection was initiated and concluded on 4/19/2022. There were 64 children present, 19 staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care provisions and emergencies, special services, and backgrounds. A total of 5 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. Staff #4 (date of employment: 11/8/21) resides outside of the state of Virginia. A sex offender registry check and a search of the child abuse and neglect registry for the state of Maryland was not obtained for Staff #4.
2. Staff #5 (date of employment: 8/4/21) resides outside of the state of Virginia. A sex offender registry check and a search of the child abuse and neglect registry for the state of Maryland was not obtained for Staff #5.
3. Staff #6 (date of employment 11/22/21) resides outside of the state of Virginia. The results of a criminal history record information check, sex offender registry check, and a search of the child abuse and neglect registry check for Washington D.C. was not obtained for Staff #6.
