Inspector Notes

An unannounced renewal inspection was initiated and concluded on 07/15/2022 between the hours of 12:35pm-2:20pm. At the time of entrance, 62 children were present. Children were observed during naptime and engaging in free play. A total of 5 child records and 3 staff records were reviewed. Agent backgrounds were also verified. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and staff interview, the child day center did not have a central registry finding within 30 days of employment for one staff.

Evidence:
1. Staff #1 began employment on 07/14/2021.
2. Documentation in the record for Staff #1 indicated a search of the central registry was submitted on 3/25/2022.
3. Staff reported the central registry check did not appear to be completed upon the 30th day of employment under previous management. Upon learning the central registry check had not been completed, a check was submitted on 3/25/2022.
Plan of Correction: Upon learning the central registry check had not been completed, a check was submitted on 3/25/2022. Documentation of follow-up inquiries made by the center was maintained in the record. The documentation has been re-sent and the center is awaiting the results.
