Based on staff interview, children under 10 years of age were not within actual sight and sound supervision of staff.
Evidence:
1. Administrative staff reported that on 2/16/2023, staff #1 was observed in the two's classroom, lying under a desk looking at an ipad while children were sleeping. The staff under the desk was not able to provide sight supervision of children as required.

Plan of Correction: Upon seeing staff lying under the desk, staff administration assigned another staff person to provide supervision in the classroom and removed the staff person from the classroom in order to provide the staff member with additional training and guidance regarding supervision.
