During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/8/2025, as a part of the licensure period. The on-site inspection began at 11:30am and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 89 children present and 16 staff. The inspector reviewed 8 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Centers are required to obtain immunization reports signed by a physician, designee, or an official of a local health department. The immunization reports in the record for Child #2 were not signed by a physician, designee, or local health department official as required.
Plan of Correction: The center will ensure immunizations obtained for children enrolled are signed by a physician.
Standard 8VAC20-780-60-A
Centers are required to keep a separate record for each child enrolled which shall contain all required information. The record for Child #2 did not contain the phone number for 1 of 2 designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The center will request the information from the parent.
