Inspector Notes

An unannounced inspection was initiated on 4/22/2025 and completed on 5/22/2025 in response to a complaint received by the licensing office on 4/09/2022 relating to supervision. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to procedures for emergencies and are documented on the violation notice issued to the program.

Centers are required to maintain a written record of children's serious and minor injuries that include all required information. Two separate injury reports documented for Child #1 included the time a parent was notified as the same time that the injury occurred. An injury report for Child #2 documented the time a parent was notified as occurring prior to the time of the incident. Staff reported the time of injury on the report is automatically populated as the time the report is generated.
Plan of Correction: Moving forward, if an injury report is printed containing an incorrect time of notification or occurrence, we will correct it by crossing out the incorrect information, adding the correct information, and initialing the correction.
