Inspector Notes

An unannounced monitoring inspection was conducted with afternoon director, who was present with 41 children and 11 staff. The children were preparing for a cooking project during the inspection. Five children and 3 staff files were reviewed as well as medications for 8 children and 6 injury/accident reports. There were no violations observed during this monitoring inspection, however, the center self reported a situation involving administering the wrong dose of medication to a child, this inspection resulted in areas of non compliance, which are outlined on the violation notice.

Standard 8VAC20-780-340-A
Based on interviews, a review of record keeping and observation, it was determined that the center failed to ensure the care and protection of a child in care.
Evidence:
On 7/13/22 staff A told a parent to sign the medication authorization form and that they would fill in all the other information required on the medication form. The parent signed the form and left and staff A proceeded to filled in the information. Staff A wrote on the form that child A was to be given 20ml of multi-symptom cold medication every 4 hours and not the 10 ml every 4 hours as directed by the parent.
The multi-symptom cold medication was given to child A ( 8 years of age) at 9 am and 1pm. The child was picked up from the summer camp at approximately 4: 45 pm by a parent, who inquired why their child looked so tired. It was then discovered that the center administered the wrong doses of medication.
Plan of Correction: This medication form was corrected on 7/14/2022 with the correct medication dosage.

Parents filled out a new medication form as needed for the child. This was corrected on 7/14/2022.

Based on interview, a review of recordkeeping and observation, it was determined that non prescription medication was given to a child not in accordance to the center's written medication policies.
Evidence:
On 7/13/22, child A was given the wrong dose of a multi symptom medication at 9 am and 1 pm. The child was given 20ml on both occasions, when in fact the child should have been given 10 ml on both occasions.
Plan of Correction: This medication form was corrected on 7/14/2022 with the correct medication dosage.

Parents filled out a new medication form as needed for the child. This was corrected on 7/14/2022.
