(Repeat Violation) Each staff member and individual from an independent contractor is required to submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

The TB screening on record for Staff #3 was dated two days after employment.

Hazardous substances such as cleaning materials are required to be kept in a locked place using a safe locking method that prevents access by children.

On 10/29/25 two cleaning sprays were observed in the windowsill of the office window. The door to the office was open and accessible to the children.

The record for Child B was not able to be located during the inspection on 10/29/25.
