Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/26/24.The on-site inspection began at 6:40 am and ended at 7:50 am. The inspector reviewed compliance in the areas listed above. There were 28 children present and 7 staff. The inspector reviewed 6 children?s records and 7 staff records on-site.
This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program..

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's injuries to include an area to document ways to prevent recurrence of the injury.

The injury/accident forms did not have a space to document ways to prevent recurrence of injuries.
Plan of Correction: We will add that statement to all injury forms.
Standard 8VAC20-780-60-A-8
A written care plan from a physician is required for each child with a diagnosed food allergy.

There was no written care plan on file for child #1 who has a diagnosed food allergy
Plan of Correction: We will obtain the written allergy plan from the physician.
