Inspector Notes
An unannounced renewal inspection was conducted on 09/07/2023 from 6:15am to 7:15am. There were 3 children ages 8 years old to age 10 years old supervised by 2 staff. The physical plant, programming, attendance, menus, 2 staff records, 3 children?s records, 3 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in trading cards and chill zone. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

1Evidence: There center did not have a current allergy list for the staff. The record for Child #1 (start date 8/28/23) stated that the child had been diagnosed with food allergies by a doctor and the information had not been added to the allergy list dated 8/18/23.
