Inspector Notes

An unannounced monitoring inspection was conducted on 03/20/24 from 7:05 a.m. to 8:05 a.m. with Site Director. There were 10 children in care, ranging in age from five to 12-years-old, supervised by two staff. The children were observed in an activity at the tables. Five child records and two staff records were reviewed. The Center has three staff with current certification in CPR and First Aid, and three staff trained in Daily Health Observation Training. Three medications and authorization forms were reviewed, and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.

Evidence: At approximately 7:05 a.m. there were five children present, the written record for the whole group reflected one, the written record for grades three through five reflected three, and the written record for kindergarten through second grade reflected zero.
Plan of Correction: We will maintain written record of our daily attendance on our white board, ipad and sign in and out sheets.
