Inspector Notes

An unannounced monitoring inspection was conducted on 04/09/2024 from 12:35pm to 2:05pm with the Director. There were 51 children ages 6 years to age 10 years old supervised by 5 staff. The physical plant, outdoor play area, programming, attendance, menus, 5 staff records, 5 children?s records, 5 medications with corresponding authorization records, 2 allergy care plans, emergency drills, 6 injury reports, emergency supplies, and policies were reviewed. Children were observed participating in an art project and outside play. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on review of 5 child records, the Center did not ensure they obtain all of the required documentation for child records.

Evidence: The record for Child #1 did not contain the name, address and phone number of 2 designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: Site Director received second emergency contact that afternoon and updated it on the form.

Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


Evidence: Upon arrival for the inspection, there were 51 children present, but the written record reflected there were 48 children present.
Plan of Correction: Site Director will ensure attendance count is accurate by ensuring children are checked in upon arrival daily.
