Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 3/30/2021. The site director was contacted by telephone to initiate the inspection. There were 7 children present and 3 staff. The inspector emailed the site director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Based on record review, the center did not obtain an updated tuberculosis (TB) screening or test for each staff member at least every two years from the date of the first initial screening or testing.
