Inspector Notes

An unannounced monitoring inspection was conducted with the director on April 12, 2024, beginning at 7:35 a.m. and ending at 8:21 a.m. There were 32 school-aged children in the direct care of 4 staff. The children were observed cleaning up art activities, hand washing, and getting breakfast. Five children?s records were reviewed and in compliance. Three staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Five medications and two allergy care plans were reviewed. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 3/22/2024. The most recent shelter-in-place drill was documented as 11/30/2023. The most recent lockdown drill was documented as 1/17/2024. The most recent health inspection was dated 10/18/2023. The most recent fire inspection was dated 10/31/2023.

Based on a review of documentation, the facility failed to obtain written authorization for all medication present at the center. Evidence: There was no written authorization for child A's medication.
Plan of Correction: The facility has previously requested this information from the parent. The staff has contacted the parent again regarding the missing documentation. A 10-day medication authorization will be signed by the parent and placed in the record.

Based on a review of documentation, the facility did not ensure that all children's records contained the required information. Evidence: Child A's record did not have a written care plan for a diagnosed food allergy.
Plan of Correction: The facility has previously requested this information from the parent. The staff has contacted the parent again regarding the missing documentation. Once the missing documentation is received it will be placed in the child's record.
