Inspector Notes
An unannounced monitoring inspection was conducted 1/30/2023 at 11:30am. There were 45 school-agers directly supervised by 6 staff. The physical plant, 3 staff records, 5 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and individual activities and games. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
1Evidence: observed that there was albuterol in the medication box that expired on 11/22. The child was no longer in the program and the medication was not returned to the parents nor was it disposed of.
