During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/19/25. The on-site inspection began at 6:35 am and ended at 7:30 am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 staff. The inspector reviewed 7 children?s records and 4 staff records on-site.This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent.

The medication authorization for child A had an effective until date of 3/4/25 and the authorization had not been renewed by the parent or physician,
Plan of Correction: I will have the parent sign the 10 day authorization with the intent on obtaining the long term authorization from the physician.
