During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/29/25. The on-site inspection began at 8:50 am and ended at 9:50 am. The inspector reviewed compliance in the areas listed above. There were 53 children present and 6 staff. The inspector reviewed 6 children?s records and 7 staff records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department shall obtain the results of a tuberculosis test/screening.

The tuberculosis tests for staff A was overdue by 3 weeks and the tuberculosis test for staff B was overdue by 1 day.
Plan of Correction: We will have the staff obtain up to date tuberculosis test/screening.
