During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/13/24. The on-site inspection began at 7 am and ended at 8:15 am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 7 children?s records and 5 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunization reports shall be signed by a physician or their designee.
Immunization records for child C, D and E were not signed by a physician or their designee. All three children have been enrolled for over a month.
Plan of Correction: We will obtain signed immunization records.
Standard 8VAC20-780-510-I
The center administers only those drugs that were dispensed from a pharmacy and mainlined in the original, labeled container. The center may administer prescription medication provided that the center has obtained written authorization from a parent/guardian.

A prescribed emergency medication was onsite for child A for over a month, this
medication did not have the original labeled container or label from the pharmacy.

Child B has been enrolled for over a month and there was no parental/guardian authorization for a prescribed medication.
Plan of Correction: We will obtain the original prescription label and authorization for the medication.
