Inspector Notes

An unannounced renewal inspection was conducted with the director on August 31, 2022 beginning at 6:50 AM and ending at 7:45 AM. There were 11 school age children in the care of 2 staff members. The children were observed playing games and doing table top activities. Six children?s records and two staff records were reviewed and determined to be in compliance. The first aid kit, flashlight, and battery-operated radio were reviewed and in compliance. Four medications were reviewed and in compliance. The most recent emergency escape drill was documented as 8/31/2022. The most recent shelter in place drill was documented as 4/5/2022.

Based on a review of documentation, the facility did not ensure that all children had documentation of immunization by first day of attendance. Evidence Child A, child B, child C, and child D did not have documentation of immunizations in their records.
Plan of Correction: The facility will request the missing documentation from the parents and update the children's records.
