Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A subsidy health and safety (SHSI) inspection was conducted on 9/15/2021. There were 18 children present with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of the written allergy list and staff interview, a written allergy care plan, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction, was not on file for all children with food allergies.

Evidence: Child #4 is allergic to nuts, peanuts, and pineapple and does not have an allergy care plan on file signed by a physician.
